FEES

Our reasonable fees are a reflective of the care, training, skills, and experience we bring to you in performing your treatment, as well as the quality of our materials and laboratory work.

INSURANCE

We work with most traditional insurance companies and offer services as an out-of-network provider for PPO Plans.  We do not participate in DMO plans.  As a courtesy to our patients, we will process and file your insurance claim.  It is essential that you provide us with the necessary information and identification in order to confirm your benefits and coverage.  Co-payments, deductibles, and co-insurance are due at the time of treatment.

FINANCING

Financial considerations should not keep you from receiving the care you need or want.  If long term financial arrangements are necessary, CARE CREDIT is available.  Care Credit is a trusted name in patient financing.  They off several payment plans to meet every financial need.  They offer short No-Interest Payment Plans as well as low interest Extended Payment Plans from $1,500 to $25,000.  You may fill out a credit application in our office or you may also apply online at www.carecredit.com .  It's easy to apply and you can receive an online decision in seconds.  Our office manager can then help facilitate arrangements for you.
